Winter Haven Hospital, part of BayCare Health System, is seeking a compassionate and skilled Outpatient Physical Therapist to join our growing rehabilitation team. In this role, you will evaluate, plan, and deliver evidence-based physical therapy services to adult and geriatric patients in an outpatient setting. You will collaborate closely with physicians, clinical staff, and interdisciplinary team members to restore function, improve mobility, and enhance quality of life for the patients we serve.

Outpatient Physical Therapist Responsibilities:

• Perform comprehensive patient evaluations and develop individualized treatment plans
• Provide therapeutic interventions including exercise prescription, manual therapy, neuromuscular re-education, and functional training
• Monitor patient progress and adjust treatment plans based on clinical outcomes
• Educate patients and caregivers on diagnosis, treatment plans, home exercise programs, and injury prevention
• Accurately document evaluations, treatments, progress notes, and discharge summaries in accordance with regulatory and BayCare standards
• Collaborate with physicians, case managers, and other healthcare professionals to ensure coordinated care
• Maintain compliance with all state, federal, and organizational policies and procedures
• Participate in quality improvement initiatives and continuing education opportunities
• Support a positive, team-oriented culture aligned with BayCare values

Requirements:

• Required BLS (Basic Life Support)
• Required PT (Physical Therapy)
• Or PT (Physical Therapy) Eligible to obtain
• Or PT (Physical Therapy) Temporary and licensure within 6 months
• Required Bachelor's Physical Therapy
• Preferred 1 year Physical Therapist
