You will be part of a multi-disciplinary team of healthcare professionals including nurses, physicians, and chaplains, all focused on delivering patient-centered care. Your work will take place primarily in the virtual realm, engaging directly with clients and families, while also collaborating effectively with those on site to ensure seamless care delivery.

Key Responsibilities\n

• Conduct comprehensive assessments of patients’ psychosocial needs, challenges, and strengths to develop individualized treatment plans that align with their goals and family dynamics.

• Facilitate therapeutic sessions utilizing a variety of modalities including cognitive-behavioral therapy, family systems therapy, and solution-focused brief therapy to enhance emotional well-being.

• Collaborate with patients and families to establish SMART (Specific, Measurable, Achievable, Relevant, Time-Bound) goals, continuously monitoring and adjusting care strategies as needed.

• Maintain detailed, confidential records in compliance with legal and ethical standards, ensuring all documentation reflects the therapeutic process and patient progress.

• Engage in ongoing professional development through training, workshops, and supervision, to stay current with trends and advancements in social work and healthcare.

Patient Population / Client Focus\n

In this role, you will primarily work with patients in hospice care, many of whom are experiencing end-of-life challenges. You will provide them with emotional, psychological, and social support, helping them navigate the complexities of their health conditions while also addressing family dynamics, grief, and adjustment issues. Your expertise will contribute significantly to enhancing their quality of life and emotional well-being during a crucial time.

Qualifications\n

• A Master’s degree or higher in Social Work from an accredited institution.

• Current active LICSW license in Massachusetts.
