PRN/Contract Nurse - In Home Client Assessments

We are seeking an experienced Registered Nurse (RN) to conduct in-home client assessments and develop individualized plans of care for our caregiving team. This is not a hands-on or bedside nursing position. Visits primarily involve assessing the client's needs, home environment, and safety. Most assessments take approximately 60 minutes, plus documentation. Responsibilities Conduct comprehensive per-visit nursing assessments including ADLs/IADLs, cognition, mobility, fall risk, home safety, medication self-administration, and dietary needs Develop, review, and update individualized plans of care that our caregivers will follow Develop, review, and update individualized care plans based on assessment findings Complete visits at required intervals and reassess clients when their needs or condition change Document assessments and care plans in AxisCare (training provided) Communicate relevant client needs or changes with our office team Schedule & Pay Set your own schedule and choose which assessments you accept. Visits are typically completed on weekdays during business hours. Flat fee per completed assessment. Independent Contractor (1099) position. Requirements Active Registered Nurse (RN) license in the state of Virginia Prior experience assessing older adults: home health, home care, or community nursing preferred Strong clinical assessment and critical thinking skills Excellent documentation skills Reliable transportation, valid driver's license, current auto insurance Ability to work independently with minimal supervision Strong interpersonal and communication skills with patients, families, and care teams Able to clear a Virginia criminal background check (required by state law for home care organizations) Must provide TB screening documentation About Us Paragon Home Care is a trusted provider of in-home care services dedicated to helping clients maintain their independence and quality of life in the comfort of their own homes.
