Key Responsibilities\n

• Conduct comprehensive assessments and develop tailored treatment plans, addressing individual needs and preferences in real time.

• Facilitate the creation of pre-crisis and crisis intervention plans, behavioral guidelines, and risk assessment methodologies.

• Prepare and complete timely documentation, strictly adhering to regulatory standards to ensure compliance and accountability.

• Implement evidence-based screening tools as required in collaboration with the clinical team, monitoring patient responses and outcomes.

• Collaborate with interdisciplinary teams to create and execute positive behavior support plans that enhance patient engagement and growth.

• Actively engage in licensing and accreditation processes, contributing to continuous improvement and quality assurance initiatives.

• Serve as a liaison between patients and relevant service systems, including state agencies and community partners, to coordinate comprehensive care.

• Maintain effective communication with various stakeholders to align services with the organization’s mission and values.

• Establish and nurture relationships with local, state, federal, and private organizations to bolster program visibility and secure potential funding opportunities.

• Perform additional duties as assigned by the Clinic Director to support operational effectiveness.
