Key Responsibilities\n

• Conduct thorough psycho-social assessments and create tailored treatment plans that align with clients' needs.

• Provide individual therapy, utilizing various therapeutic methods to address emotional and behavioral challenges.

• Maintain comprehensive and timely documentation of clinical interactions, ensuring compliance with the standards set by the organization and regulatory agencies.

• Collaborate with medical professionals to communicate clients' progress, challenges, and proposed interventions effectively.

• Facilitate crisis intervention as necessary, providing immediate support for clients experiencing acute emotional distress.

• Assist in developing and managing discharge plans, coordinating care with external resources and support systems to ensure a smooth transition to community-based services.

• Engage with clients’ families and support networks to promote understanding and collaboration throughout the treatment process.

Patient Population / Client Focus\n

The ideal candidate will work with a diverse patient population, including individuals dealing with anxiety, depression, trauma, and other mental health conditions. You will be instrumental in guiding clients through their therapeutic journeys, advocating for their needs, and empowering them to achieve their mental health goals.

Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution.

• Current licensure as a Clinical Social Worker (LCSW) in California.

• A minimum of two years of experience providing therapy in an outpatient setting.

• Proficient in using electronic medical record (EMR) systems and other digital tools that support documentation and patient management.

• Exceptional organizational skills and the ability to work independently, demonstrating self-direction in your professional practice.
