Key Responsibilities\n

• Conduct detailed psychosocial assessments by interviewing patients, observing behaviors, and reviewing medical records to identify emotional, social, and environmental factors impacting recovery.

• Deliver therapeutic counseling and emotional support to patients and their families, specifically designed to address adjustment issues related to their health conditions.

• Collaborate with the interdisciplinary rehabilitation team to create comprehensive, individualized treatment plans that integrate psychosocial factors into the care process.

• Assist patients in navigating community resources, including support groups, vocational rehabilitation programs, and housing assistance tailored to enhance their recovery.

• Educate patients and families about available resources and aftercare services to facilitate a smooth transition post-rehabilitation.

• Participate in discharge planning meetings to ensure that patients receive the necessary support and resources as they return home or transition to other care settings.

Patient Population / Client Focus\n

You will work closely with a diverse population of patients facing various challenges, including those recovering from strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. Your expertise will be essential in emotionally supporting these individuals as they regain independence and work towards reintegration into their communities.

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Active state licensure as a Licensed Clinical Social Worker (LCSW) in Kansas.

Compensation and Benefits\n
