Key Responsibilities\n

• Conduct in-depth psychosocial assessments through interviews, observational techniques, and the thorough review of medical records to understand the emotional, social, and environmental factors that may impact patients' recovery.

• Provide compassionate counseling and therapeutic support to both patients and their families, helping them navigate adjustment issues and emotional challenges during the detox process.

• Collaborate closely with the rehabilitation team to devise tailored treatment plans that address patients' unique psychosocial needs and integrate seamlessly with their medical care.

• Assist patients in connecting with community resources such as support groups, vocational rehabilitation, and housing assistance, ensuring their ongoing needs are met beyond the detox setting.

• Engage in discharge planning, facilitating safe transitions back to home or other care settings that align with the patient’s care plan.

• Educate and advocate for patients and families regarding available community resources and post-rehabilitation services that can support long-term recovery and well-being.

Qualifications\n

• Master’s degree in Social Work from an accredited institution.

• Current active state licensure as a Licensed Clinical Social Worker (LCSW).
