Support Coordinator, Child & Adolescent Developmental Services Annual Salary: $50,604Work Schedule: M-F 8:30a - 5:00p Our Organization Since 1971, the Hampton-Newport News Community Services Board has provided the highest quality of behavioral health services to residents of the cities of Hampton and Newport News, Virginia, offering a comprehensive array of programs for both children and adults living with mental illness, developmental disabilities, and/or substance use disorders.Core ResponsibilitiesThe Support Coordinator for Child & Adolescent Developmental Services ensures continuity of care for children and adolescents with intellectual and developmental disabilities by coordinating services and supporting informed, person-centered decisions.Key responsibilities include developing individualized, person-centered plans; connecting clients and families to appropriate providers and community resources; monitoring service delivery and progress toward outcomes; advocating for needs across systems; and supporting community integration across medical, educational, vocational, and recreational domains in alignment with discharge protocols.Required qualifications:- Bachelor’s degree in Human Services, such as Social Work, Psychology, or Counseling - One (1) year of experience in support coordination services with the assigned population A Typical DayYour day is primarily onsite working within a collaborative, professional team environment focused on service excellence and empathetic care. You’ll balance scheduled touchpoints with families and providers with time to update records, review plan milestones, and prepare for upcoming meetings. Throughout the day, you’ll navigate a fast-moving service landscape—shifting between problem-solving, coordinating next steps, and ensuring families have clear, practical information to make informed choices.
