Day-Shift LPN for Compassionate Long-Term Care

Job Summary Avera Morningside Heights Care Center is looking for Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide direct resident care. Each day you will positively impact each patient you serve with delivering excellent care. Work Schedule Primarily days with rotating weekends and holidays. Work shift: 7:00 am–3:30 pm, 16 hours every 2 weeks (0.2 FTE). Holiday rotation. Compensation RN Pay: $41.29–$564.16 per hour (depending on experience). LPN Pay: $24.86–$35.84 per hour. Registered Nurse (RN) Responsibilities Delivers nursing care by developing nursing diagnoses, establishing care objectives, defining nursing interventions, providing physical, emotional, psychological, and spiritual support to residents, families, and friends, and re‑evaluating care objectives. Assesses and evaluates residents within scope of practice; completes assessments, makes appropriate interventions, applies nursing process (assess, plan, implement, evaluate). Complies with pharmaceutical policies and procedures; ensures medications/treatments are given safely and accurately according to physician’s orders. Participates in the care planning process and ensures consistent, age‑appropriate resident care; initiates and updates plan of care. Communicates and manages delivery of resident care; uses SBAR communication tool during physician rounds. Processes and transcribes physician’s orders and implements them according to facility policy. Functions within the scope of the Nurse Practice Act, using recognized standards of clinical practice as defined by the State of MN. Complies with facility staffing policies, assists with data collection for quality functions, and participates in performance improvement activities. Maintains competency with operational equipment; investigates incidents, patient/family complaints and reports to VP of Resident Care or Administrator on‑call. Assists with discharge planning and education; administers medications per facility policy and is competent in Pyxis medication administration system. Registered Nurse (RN) Required Education and License Registered Nurse (RN) – Board of Nursing license in the state of practice (active license required upon hire). Registered Nurse (RN) Preferred Education and Experience 1–3 years of experience working in a clinical or long‑term care environment. Licensed Practical Nurse (LPN) Responsibilities Accountable for providing professional nursing care, assessment, and evaluations within scope of practice. Performs all licensed practical nursing functions and services as directed by the RN. Completes resident assessments, makes appropriate interventions, applies knowledge, prioritizes resident care, and uses the nursing process. Complies with pharmaceutical policies and procedures, ensuring medications/treatments are administered safely and accurately. Participates in the care planning process and implements consistent and age‑appropriate care. Initiates and updates plan of care as appropriate; communicates changes in resident condition to physician using SBAR. Processes physician orders and implements them per facility policy. Functions within the scope of the Nurse Practice Act and recognized standards of clinical practice defined by State of MN. Complies with facility staffing policies and assists with quality and performance improvement activities. Maintains operational equipment competency and investigates incidents or complaints, reporting to VP of Resident Care or Administrator on‑call. Administers medications per facility policy and is competent in Pyxis medication administration system; assists with discharge planning and education. Licensed Practical Nurse (LPN) Required Education and License Licensed Practical Nurse (LPN) – Board of Nursing license in the state of practice (active license required upon hire).
