Clinical Dietitian - Outpatient and Home Infusion

Location & Schedule Weekends PRN Remote – New Orleans. Responsibilities Introduce and explain the full scope of services provided by the Outpatient and Home Infusion Pharmacy to clinical staff. Ensure ongoing updates and education for referral sources. Coordinate clinical care from referral source into the home setting, including clarifying expectations of care in the home and medication connection at the hospital/facility to expedite discharge. Assist referral sources and patients with identification of appropriate candidates for home infusion, evaluate patient or caregiver ability to provide care, train patients on therapy and supplies. Complete nutritional assessments, prepare nutrition care plans, recommend nutritional therapies, and monitor effectiveness of nutritional care as required. Provide nutrition education to promote improved health and comply with regulatory requirements. Conduct and participate in community education, marketing, and public relations efforts. Participate in in‑service and continuing education programs. Collaborate with referral sources such as hospital case managers, home health agencies, physician offices, etc. to facilitate coordination of care for new patient referrals. Coordinate and communicate all discharge planning activities for new patient referrals with appropriate personnel and referral sources. Document records completely and accurately. Support departmental goals and adapt communication style to the specific patient population. Perform other related duties as required. Qualifications Bachelor of Science degree in Nutrition, Dietetics or related field. 1 year clinical work experience with knowledge of normal and therapeutic nutrition, nutrition screening and support principles, and modified diets. 1 year of Home Infusion experience (preferred). Registered Dietitian status with the Commission on Dietetic Registration (CDR) and current licensed Dietitian/Nutritionist status with LBEDN. Proficiency in computer skills and data entry; effective verbal and written communication; proficiency with Windows‑style applications and related software. Strong interpersonal, organizational, and prioritization skills; analytical skills to assess nutritional status and develop appropriate care plans. Commitment to professional development and evidence‑based clinical services. Knowledge, Skills & Abilities Computer skills and dexterity for data entry and information retrieval. Effective verbal and written communication skills. Proficiency with Windows applications and role‑specific software. Strong interpersonal skills. Strong personal commitment to professional development and maintaining evidence‑based clinical services. Excellent analytical skills for assessment and care planning. Exceptional organizational and prioritization skills to meet deadlines. Physical & Environmental Demands Frequently exert 10–20 pounds of force to move objects; occasionally up to 100 pounds. Risk of exposure to hazardous medications or waste through receipt, transport, storage, preparation, dispensing, administration, cleaning and disposal.
