Choose the region that best fits your lifestyle: SFO Bay Area What We Offer (Compensation & Benefits) Competitive Pay: $55.54 - $80.03 / hour (Based on experience) Schedule: Days (Monday - Friday) with rotating weekend flexibility Perks: Full comprehensive benefits package included for Full timer Your Core Responsibilities Direct Care: Provide expert physical, psychosocial, and environmental patient assessments. Treatment Plans: Design and implement personalized, highly effective patient rehabilitation plans. Collaboration: Partner seamlessly with patients, families, and internal healthcare management. Tech Integration: Use advanced computerized documentation tools to track patient success. Compliance: Maintain high productivity standards and strict infection control protocols. Education & Licenses Degree: Bachelor’s or Master’s degree from an accredited Physical Therapy program. Licensure: Active California Physical Therapist (PT) license. Mobility: Valid California Driver’s License (Class C or B) with access to a reliable vehicle. Experience Acute Care: Minimum 1 year of professional experience in an acute or similar clinical setting. Recent Practice: Direct patient care experience within the past 5 years. Preferred: 1+ years of experience in Home Health or Hospice environments. Regulatory Knowledge: Deep understanding of Title XXII, Medicare Conditions of Participation, and QI benchmarking. Financial Savvy: Strong grasp of reimbursement patterns and basic budget/financial reports. Soft Skills: Exceptional leadership, conflict resolution, negotiation, and communication skills. Adaptability: High energy, creative problem-solving, and a calm approach to changing priorities.
