Key Responsibilities

• Conduct thorough evaluations, including reviewing patient history, performing specific assessments, and documenting findings, to develop individualized treatment plans.
• Communicate evaluation results and treatment options clearly to patients and their families, incorporating their feedback and goals into the care plan.
• Implement established treatment protocols with proficiency while remaining adaptable to the patient's evolving condition and needs.
• Consult with other healthcare professionals as necessary, and document any changes in the patient's condition that may require modifications to the treatment plan.
• Manage comprehensive discharge planning and follow-up care, ensuring timely and accurate documentation of patient progress and interventions.
• Utilize tools such as electronic health records for tracking patient data, ensuring clarity and continuity in care provision.

Patient Population / Client Focus

Your work will focus on a diverse patient population, including those recovering from surgeries, managing chronic conditions, or undergoing rehabilitation following injuries. You will adapt your treatment modalities to meet the various needs of patients, from elderly clients to younger individuals requiring specialized care, all within the comforting environment of their homes.

Qualifications

• A Bachelor's Degree in Physical Therapy or an equivalent qualification.
• Current state licensure or eligibility to practice as a Physical Therapist in Florida.
• Strong interpersonal communication skills to effectively engage patients and families.
• Proficiency in evaluation techniques and developing customized treatment plans based on a variety of clinical practices.

Preferred Experience

• Previous experience in home care or outpatient settings.

Compensation and Benefits

This position offers a competitive salary range of USD 100,000 to 120,000 annually, commensurate with experience.
