Health Navigator- FQHC

Description

The Health Navigator will play a part in coordination of care patients receive in the FQHC. THe role will improve access to ongoing medical care and aid in patient retention. This position will work to improve systems of care and communication within HC programs and with external providers. This staff person will interact with patients, medical providers, case managers, counselors and other members of the treatment team and is responsible for documentation of interactions and the completion of necessary Release of Information and other documents. This position is an essential part of the integrated service delivery for our substance use, mental health and primary health care services.

ESSENTIAL FUNCTIONS:

• Provide direct support to patients receiving health services onsite at Haymarket Center.
• Provide patients with health education and coaching to manage medical issues and ensure access to care.
• Support patients through navigation of health care services.
• Collaborate with clinic staff to ensure patient access to any additional services needed.
• Provide support and advocacy to assure patients medical needs and referrals are being conveyed.
• Participate in periodic staffing to ensure follow-up on clinical care plan.
• May work with Care Coordinators from Managed Care Organizations to support access to coordinated care services.
• Assist in completing necessary documentation authorizing sharing information between Hymarket Center and outside providers.
• Gather documentation for upload into EHR to ensure compliance with requirements from fundersand regulatory bodies.
• Document interactions with staff and patients in EHR in a timely manner.
• Participate as assigned in Health & Wellness Department activities to improve health educationservices including development of materials, conducting groups and presentations or supporting other team members in such efforts.
• Maintain knowledge through training about subject matters related to job role with approval of supervisor.
• Attend all staff and scheduled supervisions
• Other duties as assigned.

Requirements

EXPERIENCE AND SKILLS:

• Bachelors degree in Social Services or health related field (Preferred).
• Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community.
• Knowledge about HIV, PrEP, Hepatitis C, diabetes, asthma, hypertension and other chronic medical conditions highly preferred.
• Knowledge of Microsoft Office program, spreadsheet management required.
• Experience working in a healthcare setting a plus.
• Previous case management experience beneficial.
• Understanding of Medicaid and navigating health insurance is highly beneficial.
• Understanding of HIPPAA, 42 CFR, AIDS Confidentiality Act required.
• Knowledge of Electronic Health Record (ECR) preferred.
• Maintain a valid Illinois Driver's License and eligibility for agency insurance coverage.
