Job Summary:
Caledonia Home Health & Hospice, a division of Northern Counties Health Care, is seeking a compassionate and team-oriented Registered Nurse (RN) to provide comprehensive home health and hospice nursing services to patients of all ages. The RN will be responsible for patient admissions, assessments, care planning, and coordination of care while working collaboratively with an interdisciplinary team of healthcare professionals and paraprofessionals.

Key Responsibilities:

• Conduct patient admissions, assessments, and ongoing evaluations.

• Develop, implement, and update individualized care plans.

• Provide skilled nursing care in home health and hospice settings.

• Monitor patient progress and coordinate care with physicians and healthcare team members.

• Educate patients and families regarding treatment plans and disease management.

• Maintain accurate and timely clinical documentation.

• Ensure high-quality, patient-centered care and compliance with agency policies.

Requirements

Required Qualifications:

• Active Vermont Registered Nurse (RN) License.

• Minimum one year of clinical nursing experience.

• Strong assessment, communication, and organizational skills.

• Ability to work independently and as part of a multidisciplinary team.

Preferred Qualifications:

• Previous Home Health and/or Hospice experience.

• Experience with electronic medical records (EMR).

Required Licenses/Certifications Prior to Start:

• Current Vermont RN License.

• Valid BLS Certification (if required by facility).
