Key Responsibilities\n

• Conduct thorough assessments of patients’ psychosocial needs to formulate appropriate treatment plans, incorporating case management, short-term counseling, and crisis intervention as necessary.

• Guide patients through the completion of essential forms for entitlements and supplemental insurance, facilitating access to benefits that reduce reliance on emergency assistance programs.

• Provide assistance in securing prescription cost relief through indigent programs and other community resources, improving medication adherence and health outcomes.

• Act as a patient advocate in engagements with community agencies, ensuring that clients receive necessary resources and support.

• Maintain detailed documentation of patient records, including reports and treatment plans, in compliance with HIPAA regulations and organizational standards.

• Coordinate transportation assistance for clients when required, ensuring they can attend appointments and receive necessary care without barriers.
