Key Responsibilities\n

• Conduct thorough psychosocial assessments of patients to identify emotional, social, and financial needs tailored specifically to individuals in a hospice setting.

• Provide individual and group therapy sessions to help patients and family members navigate complex feelings surrounding end-of-life care, adjustment to grief, and coping strategies.

• Engage with medical teams regularly to devise and implement holistic treatment plans that address both medical and psychosocial aspects of patient care.

• Coordinate with community organizations and services to facilitate seamless post-discharge plans and ensure continuity of care.

• Educate patients and families about various treatment options, available resources, and coping mechanisms to enhance their understanding and engagement in care.

• Develop comprehensive discharge plans that connect patients and families to appropriate community resources prior to leaving hospice care.

• Regularly participate in interdisciplinary meetings, contributing your social work expertise to improve team-based approaches and patient outcomes.

• Document all patient interactions and treatment plans accurately and timely to maintain compliance with legal and hospital standards.

Patient Population / Client Focus\n

This role specifically focuses on patients receiving hospice care and their families, addressing sensitive issues such as terminal illness, grief, and emotional trauma. You will work collaboratively with teams to ensure that the patients’ psychological needs are recognized alongside their medical needs, making a meaningful impact during one of life’s most challenging moments.

Qualifications\n

• Master's degree in Social Work from an accredited program.

• Current, active state licensure as a Licensed Clinical Social Worker (LCSW) is a must.
