Concept Pharmacy
Active Pennsylvania Pharmacist license required
Concept Pharmacy is hiring a Full-Time Clinical Pharmacist to support our long-term care operations. This is a non-retail, clinically focused role within a Joint Commission–accredited pharmacy serving skilled nursing, assisted living, and home care facilities across Pennsylvania.

Established, accredited pharmacy with 25+ years in LTC
Key Responsibilities
Verify, process, and dispense medication orders
Supervise IV admixture preparation in sterile compounding room
Conduct comprehensive medication regimen reviews
Oversee medication inventory management and delivery coordination
Serve as clinical resource for facility staff and providers
Required Qualifications
Active, unrestricted Pennsylvania Pharmacist license (RPh or PharmD)
Prior experience practicing as a licensed pharmacist in long-term care, institutional, or community pharmacy
Working knowledge of IV compounding and USP standards
Strong clinical judgment and ability to work independently in a fast-paced environment
