Key Responsibilities\n

• Conduct thorough assessments of patients’ psychosocial needs, evaluating their mental health conditions and determining the best treatment or referral options, including short-term counseling and crisis intervention.

• Guide patients through necessary paperwork for entitlements, supplemental insurance, and other financial assistance programs, working to alleviate their dependency on emergency assistance.
\n
• Facilitate access to prescription cost assistance programs by collaborating with community resources and pharmaceutical companies to alleviate medication expenses for our patients.
\n
• Serve as an advocate for patients, effectively navigating interactions with various community agencies to ensure they receive the support needed.
\n
• Maintain meticulous and accurate patient records, documenting assessments, treatment plans, and progress in compliance with organizational standards.
\n
• Coordinate transportation assistance for patients when necessary, ensuring they can attend in-person appointments or access community services.
\n
• Participate in clinical team meetings to discuss case progress and contribute to case conferences, fostering collaboration and comprehensive care strategies.

Patient Population / Client Focus\n

You will work with a diverse clientele within San Bernardino, ranging from children to adults, each facing unique challenges such as anxiety, depression, PTSD, and other mental health disorders. Your interventions will be tailored to meet the specific needs of each individual, promoting recovery, resilience, and empowerment in a supportive therapeutic environment.

Qualifications\n

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required.

• Possession of a current and valid license as an LMFT, LPCC, or LCSW.

• Minimum of two (2) years of professional experience providing mental health counseling in a similar setting.

Preferred Experience\n

While not mandatory, experience utilizing telehealth technologies for therapy sessions and familiarity with various electronic health record (EHR) systems will be viewed favorably. Furthermore, having a background in community resources and case management will enhance your effectiveness in this role.

Compensation and Benefits\n
