We are seeking a start of care nurse to cover the twin cities metro area.

We offer hourly pay — not per visit — so you’re compensated for all the time and effort you put into thorough assessments, complex documentation, and delivering exceptional patient advocacy.

Our interdisciplinary team provides Physical, Occupational, and Speech Therapy, and we are expanding our nursing leadership to better serve our clients in group homes and home health settings.

What You’ll Do

• Conduct comprehensive Start of Care (SOC) assessments and admissions.
• Develop individualized, holistic plans of care in collaboration with the clinical team.
• Perform routine nursing assessments and medication reconciliations.
• Coordinate with physicians, families, and group home staff to ensure seamless care transitions.

What You Bring

• Current MN Registered Nurse (RN) License.
• A "big picture" mindset—you enjoy the puzzle of a complex admission.
• Strong rapport-building skills with a diverse caseload.
• Excellent clinical documentation skills and attention to detail.
• OASIS documentation experience beneficial
