Job Title: Licensed Practical Nurse (LPN) – Skilled Nursing Facility (SNF) (All shifts needed, every other weekend). Location: Woodmere, NY 11598 Job Type: LPN full time, 40+ hrs weekly, Starts ASAP, ongoing assignment, every other weekend required. 24-48 hour onboarding if documents ready. Rate: $45 hourly, weekly pay. Position Overview As an LPN in our skilled nursing home facility, you will provide high‑quality care to residents, administering medications, monitoring health, assisting with treatments, and collaborating with the multidisciplinary team. Key Responsibilities Administer medications and treatments as prescribed by physicians. Monitor and document resident health status and communicate changes to appropriate team members. Assist with activities of daily living and ensure residents' comfort and safety. Collaborate with physicians, nurses, and other healthcare professionals to develop and implement care plans. Maintain accurate and up-to-date resident records and charts. Provide emotional support to residents and their families. Qualifications Valid LPN license in the state of New York. Compassionate and empathetic attitude toward seniors. Strong communication and teamwork skills. Ability to adapt to various situations and prioritize effectively.
