Key Responsibilities\n

• Offer one-on-one therapeutic counseling to students and their families, providing support for adjustment issues, emotional resilience, and coping strategies tailored to individual needs.

• Work collaboratively with multidisciplinary teams to develop comprehensive, individualized treatment plans that address the psychosocial aspects of rehabilitation.

• Assist students in accessing community resources, such as support groups and vocational rehabilitation, to promote successful reintegration and continued growth.

• Create educational materials and conduct workshops for families, focusing on navigating community resources and understanding post-rehabilitation support services.

• Engage in proactive discharge planning and facilitate smooth transitions for students returning to home or alternative care settings, ensuring continuity of care.

Qualifications\n
