Case Manager - Vocational Rehabilitation

Department: Center for Recovery & Wellness - Vocational Services (New York Region)
Schedule: Monday - Friday | 9:00 AM - 5:00 PM
Status: Full-Time | Union | Non-Exempt (37.5 hours/week)

As a Substance Use Disorder (SUD) Vocational Rehabilitation Counselor , you will play a vital role in empowering individuals in recovery by providing career counseling, employment support, and job placement services that foster long-term independence and success.

Working within a collaborative clinical environment, you'll partner with treatment teams, employers, and community organizations to create opportunities that transform lives while promoting recovery-friendly workplaces throughout Manhattan.

Key Responsibilities

Direct Client Services

• Conduct comprehensive employment readiness and vocational assessments for new clients.
• Develop individualized, person-centered employment and career development plans.
• Provide ongoing one-on-one vocational counseling and facilitate group vocational therapy sessions.
• Assist clients with resume development, job applications, interview preparation, and employment readiness skills.
• Coordinate job matching and placement activities while conducting retention follow-ups at 6-day, 30-day, 60-day, and 90-day milestones.

Employer & Community Engagement

• Build and maintain strategic partnerships with local employers across diverse industries.
• Conduct outreach and educate businesses on recovery-friendly workplace practices to reduce stigma surrounding substance use disorders.
• Collaborate with OASAS job development resources and participate in statewide workforce initiatives.

Regional Coordination & Integrated Care

• Deliver mobile and virtual vocational rehabilitation services across OASAS-certified residential (Part 820) and outpatient (Part 822) programs and partner agencies throughout Manhattan.
• Collaborate closely with clinical providers and case management teams to ensure coordinated, holistic care.
• Participate in monthly meetings with the OASAS Technical Assistance Liaison to align services and best practices.

Documentation & Compliance

• Maintain accurate client tracking using unique identifiers while ensuring zero personally identifiable information (PII) is documented.
• Complete monthly reporting on vocational assessments, job search activities, placements, and retention outcomes.
• Ensure full compliance with 42 CFR Part 2 , HIPAA , and OASAS regulations.
• Verify that vocational services remain non-duplicative of other state-funded programs.

Qualifications

Required

• Master's Degree in Rehabilitation Counseling or General Counseling .
• Minimum of 1 year of career counseling experience working with individuals with substance use disorders (SUD) .
• Experience with or credentials related to ACCES-VR (Adult Career and Continuing Education Services - Vocational Rehabilitation) .
• Knowledge of New York workforce development systems.
• Strong understanding of:
  • Strengths-based counseling
  • Cognitive Behavioral Therapy (CBT)
  • Behavior modification techniques
• Working knowledge of 42 CFR Part 2 , HIPAA , and OASAS regulations.
• Experience using Electronic Health Records (EHR), Relias, and Microsoft Office Suite (Word, Excel, Outlook).

Preferred

• Certified Rehabilitation Counselor (CRC) credential.
