Nurse Practitioner

Seeking a compassionate Nurse Practitioner to provide high-quality, value-based primary care to adult and geriatric patients in an outpatient setting. The provider will manage acute and chronic conditions, perform preventive care, coordinate with interdisciplinary teams, and educate patients to improve overall health outcomes. Key Responsibilities Provide comprehensive primary care for adult patients. Conduct wellness exams, preventive visits, and health assessments. Diagnose and treat acute and chronic conditions. Prescribe medications and develop individualized treatment plans. Coordinate care with specialists and other healthcare providers. Educate patients on disease prevention and health management. Support care coordination, patient outreach, and follow-up. Perform home visits as needed. Qualifications Master's or Doctorate in Nursing, or Nurse Practitioner Certificate. National certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care NP (AGPCNP), Adult NP (ANP), or Gerontological NP (GNP) . Active DEA license. Current authorization to work in the U.S. Spanish language proficiency is required. Preferred Experience in Primary Care, Family Medicine, Internal Medicine, or Geriatrics. Knowledge of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs). Strong communication, teamwork, and patient-centered care skills. #J-18808-Ljbffr Aimic Inc
