Horizon Post Acute is seeking a highly motivated and detail-oriented MDS Coordinator to join our team! As an integral part of our healthcare organization, you will play a crucial role in ensuring accurate and efficient completion of Minimum Data Set (MDS) assessments, care plans, and care coordination.

About the Role:

As an MDS Coordinator, you will be responsible for:

• Conducting MDS assessments, developing and implementing care plans, and coordinating care with interdisciplinary teams

• Assuring accurate and thorough documentation of patient assessment data, care plans, and care coordination activities

• Identifying and addressing potential barriers to care, and implementing strategies to improve patient outcomes

• Collaborating with healthcare professionals, caregivers, and families to ensure comprehensive and individualized care

• Maintaining accurate and up-to-date records, reports, and statistical data

• Participating in quality improvement initiatives to enhance patient care and outcomes

Responsibilities:

• Coordinate and conduct MDS assessments, care planning, and care coordination activities

• Ensure accurate and timely completion of MDS assessments and care plans

• Develop and implement effective care plans that address patient needs and outcomes

• Collaborate with healthcare professionals, caregivers, and families to ensure comprehensive and individualized care

• Identify and address potential barriers to care, and implement strategies to improve patient outcomes

• Maintain accurate and up-to-date records, reports, and statistical data

Requirements:

• Bachelor's degree in a related field (e.g., nursing, social work, healthcare administration)

• 1+ years of experience in MDS coordination, healthcare administration, or a related field

• Strong analytical, communication, and organizational skills

• Ability to work effectively in a fast-paced environment with multiple priorities

• Strong attention to detail and accuracy in data collection and documentation
