Sexual Health Navigator - HIV/STI Screening & Outreach

Fenway Community Health in Boston, MA is seeking a Health Navigator focused on Sexual Health to provide outreach, risk-reduction counseling and HIV/Hepatitis C/STI screening services, check-ins, and linkage to care for MSM, transgender individuals and PWID. You will work with clinicians, nurses and outreach teams, document results in Epic, perform phlebotomy, provide non-judgmental care, and help connect patients to the full spectrum of services through referrals. #J-18808-Ljbffr Fenway Community Health
