The Community Health Clinic setting will be integrated into your work, where you will collaborate with a diverse team of healthcare professionals. Key Responsibilities\n

• Conduct thorough assessments of clients experiencing symptoms of major mental illness and severe emotional disorders, utilizing evidence-based screening tools to determine their needs.

• Develop and implement integrated care plans that consider the unique circumstances of each client, ensuring access to linked services and fostering communication among clients, their families, and other providers.

• Deliver focused prevention, treatment, and recovery services, including individual, group, and family therapy sessions tailored to each client's distinct needs.

• Monitor client progress, ensuring adherence to treatment plans and making necessary adjustments based on ongoing assessments.

• Assist in crisis intervention efforts, stepping in to provide immediate support and developing safety plans as needed.

• Participate in team efforts to optimize service delivery and contribute to quality improvement initiatives within Lifeline Community Care.

• Maintain timely documentation and reporting of client interactions, session outcomes, and program metrics to comply with organizational standards.

• Provide on-call coverage during and after program hours, ensuring continuous support for clients.

Patient Population / Client Focus\n

Your role will focus primarily on clients who may have limited access to mental health resources, often characterized by diverse backgrounds and varying levels of support. Many of our clients experience complex, multifaceted challenges, and require holistic approaches to therapy that consider social determinants of health. By working within the community, you will not only address mental health needs but also connect clients to broader health and social services that enhance their overall well-being.

Qualifications\n

• Master's degree in Social Work from an accredited program with a valid LCSW license in the state of Florida.

• A minimum of two years of direct experience in clinical social work, particularly in mental health settings.

• Expert knowledge in assessment, diagnosis, treatment planning, and crisis management for individuals dealing with a range of emotional disorders.

• Strong verbal and written communication skills to facilitate effective interactions with clients, team members, and other stakeholders.

• Proficiency in organizing and managing multiple caseloads while maintaining thorough documentation.

Compensation and Benefits\n

This full-time position offers a competitive salary range of $70,000 to $85,000 annually, commensurate with experience.
