What You'll Do

As the MDS Coordinator, you will play an essential role in coordinating resident assessments and ensuring high-quality, individualized care.

Your responsibilities will include:

• Coordinating and completing the MDS assessment process in accordance with state and federal regulations
• Developing and maintaining individualized resident care plans alongside the interdisciplinary team
• Ensuring all MDS documentation is accurate, complete, and submitted on time
• Monitoring assessment schedules and important regulatory deadlines
• Reviewing clinical documentation to support quality outcomes and appropriate reimbursement
• Collaborating with nursing, therapy, dietary, social services, and other departments
• Supporting survey readiness and ongoing regulatory compliance
• Helping ensure the highest standards of resident-centered care throughout the facility
• Maintaining compliance with facility policies and procedures
• Performing other duties as assigned

What We're Looking For

• Current Colorado RN or LPN license in good standing, or compact license if applicable
• Current CPR Certification
• Previous MDS experience preferred
• Long-term care experience preferred
• Strong organizational and time-management skills
• Excellent communication and documentation abilities
• Ability to work effectively with an interdisciplinary team
• A passion for providing exceptional care and improving the lives of residents
