Wellness Nurse, LVN -Full Time

Position: Wellness Nurse Pay Range: $35.00-$45.00 per hour. Ivy Park at Belmont is a premier senior living community managed by Oakmont Management Group. Coordinate incidental medical, dental, vision, hearing, and podiatry care for residents. Perform nursing duties as directed and within the scope of the Nurse Practice Act, including medication administration and treatments ordered by a physician. Provide support to Medication Technicians to ensure competent delivery of medications. Assist with resident assessment tasks as directed by the Health Services Director. Conduct staff interviews and observe residents continuously to inform staff of changes in physical and emotional health; communicate information to physicians and other healthcare professionals. Complete thorough documentation in the resident chart per company policy and state regulations. Review and monitor staff use of the Shift Report daily; participate in the change‑of‑shift meeting to communicate pertinent resident care information. Coordinate prescription orders with doctors’ offices and pharmacy delivery of medication; oversee the central storage and delivery of medications. Communicate with residents and responsible parties regarding the community’s preferred pharmacy and incontinence supply programs. Qualifications Licensed Vocational or Registered Nurse with a valid California nursing license in good standing. Prefer one year of experience working in a Residential Care Facility for the Elderly (RCFE). Proficient in operating a personal computer and MS Office programs (Word, Excel, etc.). Able to use standard office equipment, including fax machine, copier, and telephone. Knowledge of, or willingness to learn, assisted living and dementia care practices. Ability to work patiently with seniors and cognitively impaired individuals. Valid first aid and CPR certification. Must pass a criminal background check and health screening tests, including physical and TB tests.
