Work Environment\n

This LCSW position is primarily remote, allowing you the flexibility to work from your home while still contributing to essential services for our patients. However, you may occasionally visit our hospice center in Pembroke Pines, Florida, to conduct in-person assessments or participate in collaborative meetings with healthcare teams. Key Responsibilities\n

• Conduct comprehensive evaluations and assessments for patients and families, identifying needs and developing appropriate intervention strategies.

• Collaborate with interdisciplinary teams to create and implement Individualized Family Service Plans (IFSPs) tailored to the needs of each patient.

• Monitor progress and outcomes, providing feedback and adjustments to care plans as necessary to ensure optimal patient development.

• Provide emotional support and guidance to families, assisting them in navigating the complexities of hospice care.
\n
• Ensure compliance with all relevant federal and state regulations, maintaining a high standard of care and service delivery.

• Utilize evidence-based interventions and Natural Learning Environment Practices to foster an enriching environment for patients.
\n
• Maintain accurate and up-to-date records of patient interactions and care plans in our designated case management system.
\n
• Engage in continuous professional development and training to stay abreast of best practices in social work and healthcare.

Patient Population / Client Focus\n

The LCSW will primarily work with patients and families receiving hospice care, including individuals with terminal illnesses and their loved ones. This role focuses on providing emotional and psychological support, addressing the unique challenges faced by families during this sensitive time. Your expertise will guide families in understanding the care process, coping with grief, and finding resources to support their well-being.

Qualifications\n

• A Master’s or Doctorate in Social Work from an accredited institution.

• Current licensure from the state’s Department of Public Health as a Licensed Clinical Social Worker (LCSW).

• A valid driver’s license to facilitate necessary travel for in-home visits and meetings.

Compensation and Benefits\n
