Key Responsibilities\n

• Conduct comprehensive assessments and ongoing evaluations to tailor treatment plans that address the unique needs of each client, ensuring effective progress is made.

• Collaborate closely with supervisors, direct care staff, families, and guardians to share updates regarding client progress and to coordinate treatment efforts efficiently.

• Draft and maintain a tentative schedule of appointments, home visits, and necessary observations, facilitating seamless monitoring of client advancements.

• Organize and oversee home visits and in-home observations, ensuring that you conduct thorough chart reviews and data analyses related to client care.
\n
• Guarantee that treatment plan implementations are effective, adhering to best practices and maintaining high standards in service delivery.
\n
• Participate actively in Interdisciplinary Team Meetings and other essential discussions to enhance client care and operational effectiveness.
\n
• Prepare monthly and quarterly progress notes that document client progress and contribute to the continuity of care.
\n
• Design and deliver training sessions to ensure staff and team members are well-versed in program plans and documentation procedures.
\n
• Provide follow-up support through interviews and additional visits to ensure clients are receiving the care they require.
\n
• Maintain accurate and comprehensive records that are readily accessible for authorized parties, safeguarding client information in compliance with ethical standards.

• Act as a liaison between the mental health community, public stakeholders, and clients to foster program development and enhance community collaboration.

• Contribute to creating training materials and procedures aimed at continuous improvement of our treatment programs.

The ideal candidate will work with a diverse patient population, including individuals of all ages, from adolescents grappling with anxiety and depression to adults facing complex life challenges. Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related field is required, establishing a strong foundation for effective practice.

• Active and unrestricted licensure as a Professional Counselor in New Jersey is mandatory.

Preferred Experience\n
