Care Manager

Long-Term Service Care Manager 6-Month Contract | Potential Extension/Conversion FTE Shift : 8:00 AM – 5:00 PM Remote with member facing within county (3 days home visits - 2 days work from home) We’re looking for a dedicated Long-term Service Care Manager to join our team. In this role, you’ll manage a caseload of members with long-term care needs, complete comprehensive assessments, and coordinate health services to ensure seamless, person-centered care. Key Responsibilities: Conduct monthly and quarterly member contact, including in-person visits (80% travel) Perform in-depth member assessments and document care notes Collaborate with caregivers, providers, and discharge planners Monitor services, follow up with members, and coordinate referrals Assist members with complaints and appeals Qualifications: Bachelor's degree and 2–4 years of related experience 2+ years in Care Management (LTSS, Home Health, or Discharge Planning) Familiarity with electronic medical health records and Microsoft Office Valid driver’s license (Travel Required) Bilingual (Spanish) preferred Applicant Notices & Disclaimers: For information on benefits, equal opportunity employment, and location-specific applicant notices, click here .
