Hospice NurseWe are seeking a dedicated Hospice Nurse to provide comprehensive end-of-life care to terminally ill patients, adhering to hospice philosophy and regulatory guidelines. This role involves managing a caseload of patients through various nursing interventions aimed at addressing their physical, emotional, and psychosocial needs.Key ResponsibilitiesConduct thorough assessments of patients to determine their care needs.Develop individualized care plans tailored to each patient's preferences and requirements.Provide skilled nursing interventions and symptom management to enhance patient comfort.Administer medications as prescribed and monitor treatment effectiveness.Educate patients and their families about care options, medication management, and coping strategies.Act as the primary point of contact for patients and families, fostering seamless communication among the interdisciplinary hospice team.Coordinate with physicians and support staff to ensure comprehensive and continuous care.Evaluate and reassess patient care plans and interventions regularly to meet changing needs.QualificationsRegistered Nurse (RN) licensure valid in the applicable state.Experience in hospice, palliative care, or related fields preferred.Strong assessment, communication, and interpersonal skills.Ability to work collaboratively within an interdisciplinary team.Compassionate and patient-centered approach to care.LocationThis position is available in various locations within the region, including Dallas, TX.
