Shift: 8:00 AM - 4:30 PM

Assignment Details

Guaranteed Hours: 36.00, Contract Weeks: 13

Requirements

• Bachelor's Degree in Nursing (BSN): Preferred
• Five years' hospice nursing experience: Preferred
• Registered Nurse (RN) licensure in the state of practice: Required
• Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification: Required
• Valid Driver's License and must be at least 21 years of age: Required
• Current automobile insurance and a reliable automobile: Required

Responsibilities

• Coordinate and deliver comprehensive end-of-life care to terminally ill patients in alignment with hospice philosophy and regulatory guidelines.
• Manage a caseload of patients, conducting thorough assessments, developing individualized care plans, and providing skilled nursing interventions.
• Serve as the primary point of contact for patients, families, and the interdisciplinary team, ensuring seamless communication and continuity of care.
• Handle symptom management, medication administration, and patient education.
• Participate in interdisciplinary team meetings to review and revise care plans and ensure compliance with all regulations.
• Participate in the on-call rotation to respond to after-hours patient needs.
