Responsibilities
- Coordinate and deliver comprehensive end-of-life care to terminally ill patients in accordance with hospice philosophy and regulatory guidelines.
- Manage a caseload of patients, conducting thorough assessments and developing individualized care plans.
- Provide skilled nursing interventions to address physical, emotional, and psychosocial needs; manage symptoms and medications as appropriate.
- Serve as the primary point of contact for patients, families, and the interdisciplinary hospice team; ensure clear communication and continuity of care.
- Educate patients and families on disease processes, care options, and self-management; collaborate with physicians and support staff to evaluate care effectiveness.

Requirements
- Active RN license in California (or eligible for licensure in the assignment state)
- Experience in hospice, palliative care, or case management environments preferred
- Strong assessment, care planning, and documentation skills
- Excellent communication and collaboration abilities with patients, families, and interdisciplinary teams
- Ability to work effectively in a hospital-based hospice setting and comply with regulatory guidelines
