Pharmacist - Los Angeles, California

A clinical pharmacist role is available in Van Nuys, CA, offering a 13-week contract-to-perm opportunity with a focus on long-term career growth. Ideal for bilingual candidates fluent in Spanish and English, this position is embedded in a team-oriented environment that values clinical excellence and patient-centered care.


Responsibilities:

• Conduct retail pharmacy duties with a strong clinical focus, including medication review and patient counseling, particularly for a large diabetic population.
• Verify and dispense around 250 prescriptions daily, maintaining accuracy, safety, and regulatory compliance.
• Perform comprehensive clinical medication reviews assessing appropriateness, interactions, dosing, and therapeutic outcomes.
• Provide in-depth patient counseling on diabetes management, including insulin use, injectables, continuous glucose monitors, oral agents, and lifestyle modifications.
• Conduct clinical assessments and monitor chronic disease states with a focus on diabetes.
• Collaborate with prescribers to clarify orders, recommend therapy adjustments, and optimize patient outcomes.
• Identify and resolve medication-related issues and adherence barriers.
• Oversee pharmacy workflow to ensure smooth operations and regulatory compliance.
• Support a patient-centered environment committed to long-term health outcomes and quality care.

Qualifications:

• B.S. or PharmD from an accredited Pharmacy School.
• Active and clear California Pharmacist License.
• Minimum of 2 years clinical pharmacy experience preferred.
• Experience in diabetic clinical assessments highly desirable.
• Bilingual fluency in Spanish and English strongly preferred.
• Demonstrated reliability, professionalism, strong clinical knowledge, and excellent counseling skills.

Schedule:

• Monday through Friday, 9:00 AM to 6:00 PM
• Rotating every 3rd Saturday, 9:00 AM to 1:00 PM
