Key Responsibilities\n

As a Licensed Professional Counselor at Open Doors Counseling, your primary duties will include:

• Conducting comprehensive assessments of clients to identify their unique mental health needs, ensuring personalized care plans.

• Actively collaborating with supervisors, care staff, and family members to communicate progress and coordinate care effectively.

• Developing and maintaining tentative schedules that outline appointments, home visits, and monitoring activities to track client advancement.

• Coordinating necessary home visits to fulfill in-home observations, data analysis, and staff interviews as part of the assessment process.

• Ensuring the effective implementation of individualized care plans and monitoring their efficacy.
\n
• Participating in Interdisciplinary Team Meetings and other relevant discussions to align strategies for patient care.
\n
• Documenting monthly and quarterly client progress notes to keep records current and accessible.
\n
• Conducting ongoing training sessions for staff, ensuring that all team members are proficient in care plan implementation and documentation requirements.

• Following up with clients through interviews and home visits to offer continuous support tailored to their evolving needs.

• Maintaining accurate records and documentation, ensuring compliance with legal and ethical standards in mental health.

• Serving as a liaison with the mental health community to promote developmental goals and community resources.

Qualifications\n
