Key Responsibilities\n

Your duties as a Licensed Clinical Social Worker will encompass a variety of tasks aimed at fostering the well-being of our senior clients:

• Conduct comprehensive assessments to evaluate the psychosocial needs of seniors, including emotional, social, and mental health factors.

• Develop personalized care plans in collaboration with clients and their families to address their specific goals and preferences.

• Provide counseling services and emotional support to seniors and their families, guiding them through transitions or challenges they may face.

• Advocate for clients' rights and access to vital community resources, ensuring they receive the support they need.

• Engage with interdisciplinary teams, including nurses and healthcare providers, to coordinate care and improve service delivery to clients.

• Regularly visit senior clients to monitor their conditions, assess living environments, and address any arising concerns.

• Facilitate communication among family members involved in the senior clients' care to resolve conflicts and support collaborative decision-making.

• Assist clients and families in identifying available community resources, support groups, and programs that enhance social engagement.

• Maintain accurate and complete documentation, including client assessments, care plans, and progress notes that comply with professional standards.

• Stay informed about best practices, policies, and ethical standards relevant to social work.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A current, valid state license as a Clinical Social Worker (LCSW) is required.

Compensation and Benefits\n

The salary for this full-time position ranges from USD 60,000 to 100,000 annually, commensurate with experience.
