This remote pharmacist opportunity focuses on Medication Therapy Management (MTM) consulting within a telehealth environment, offering a 13-week full-time contract with flexibility to work multi-state. Licensed pharmacists in Maine, North Dakota, or Louisiana are encouraged to apply, with Louisiana-licensed candidates having priority due to a high volume of patient care anticipated during project rollout.

Key Responsibilities:

• Conduct virtual patient consultations via phone or video, performing Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews (TMRs) to address side effects, clarify therapy questions, and improve medication adherence.
• Evaluate utilization management requests including prior authorizations, formulary exceptions, and step-therapy limits against clinical and safety guidelines.
• Collaborate with prescribers through secure communication channels to recommend cost-effective, formulary-aligned medication alternatives.
• Proactively identify and reach out to patients and providers regarding gaps in care, ensuring optimal management for chronic conditions such as diabetes and cardiovascular disease.
• Assist in formulary support by translating clinical intent into coding rules for pharmacy benefit management claims systems and reviewing complex claims data.
• Maintain compliance with HIPAA, state telepharmacy regulations, and client-specific contractual requirements, documenting all activities accurately.

Qualifications / Desired Experience:

• Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy degree.
• Active, unrestricted pharmacist license in at least one state among Maine, North Dakota, or Louisiana; multi-state licensure preferred.
• Minimum 2 years of clinical pharmacy experience, ideally within PBM, managed care, or telehealth clinical settings.
• Strong clinical knowledge in chronic disease management, drug utilization review, and formulary navigation.
• Proficiency with telehealth platforms, electronic health records (EHR), and pharmacy claims adjudication software.
• Excellent communication skills for sensitive patient interactions and professional provider collaboration.
