This remote pharmacist opportunity focuses on Medication Therapy Management (MTM) consulting within a telehealth environment. The role is full time for 13 weeks with the possibility of extension, offering dynamic clinical engagement across multiple states including Maine, North Dakota, and Louisiana. Pharmacists licensed in Louisiana will have increased access to full-time assignments as this MTM project expands to serve approximately 250,000 lives this fall.

Key Responsibilities:

• Conduct virtual patient consultations through telephonic or video-based Comprehensive Medication Reviews (CMRs) and Targeted Medication Reviews (TMRs), addressing side effects, therapy questions, and adherence.
• Evaluate prior authorizations, formulary exceptions, and step-therapy limits following clinical guidelines and safety standards for utilization management.
• Collaborate with prescribers and care teams using secure communication to recommend cost-effective, formulary-aligned alternatives for restricted or non-formulary drugs.
• Proactively identify and reach out to members and their providers to close gaps in care relating to chronic disease management.
• Assist with formulary support by translating clinical intent into rules for claims adjudication systems and reviewing complex claims data.
• Maintain compliance with HIPAA, state telepharmacy regulations, and client contractual requirements through accurate documentation of clinical decisions.

Qualifications:

• Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy degree.
• Active, unrestricted pharmacist license in one of the specified states, with multi-state licensure preferred.
• Minimum of 2 years clinical pharmacy experience, preferably in PBM, managed care, or telehealth settings.
• Strong clinical knowledge in chronic disease management, drug utilization reviews, and formulary processes.
• Proficient with telehealth technology, electronic health records, and pharmacy benefit management claims platforms.
• Excellent verbal and written communication skills for patient interactions and professional provider communications.
