Key Responsibilities\n

• Conduct thorough psychosocial assessments to capture the needs and strengths of each client.

• Develop and implement individualized, strengths-based treatment plans that align with evidence-based practices.

• Provide individual, family, and group therapy sessions using therapeutic modalities such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and trauma-informed care.

• Coordinate care with interdisciplinary team members, including psychiatrists, case managers, and community resources to ensure comprehensive support for clients.

• Maintain accurate and timely clinical documentation in compliance with Medi-Cal and ethical standards, facilitating effective communication within the care team.

• Participate in regular case consultations and team meetings, sharing insights and seeking collaborative solutions for complex cases.

• Engage in advocacy and crisis intervention as necessary, empowering clients to navigate their social systems effectively.

Patient Population / Client Focus\n

As an LCSW, you will have the opportunity to work with a diverse range of clients, particularly within the hospice setting. Our patient population includes trauma survivors, youth, adults with co-occurring disorders, marginalized groups, and individuals navigating significant life transitions. Qualifications\n

• Must hold an active Licensed Clinical Social Worker (LCSW) license in good standing in California.

• A Bachelor’s degree in Social Work or a related field is required.
