Key Responsibilities\n

• Conduct thorough clinical evaluations to assess patients' psychological and emotional needs, developing tailored care plans in partnership with the healthcare team.

• Lead individual therapy sessions and facilitate psychoeducational and process-oriented therapy groups to empower patients and families in coping with the challenges of terminal illness.

• Ensure discharge appointments are confirmed with patients and their loved ones, providing necessary resources and guidance to facilitate a smooth transition.

• Maintain active communication with external referral sources and outpatient providers to ensure continuity of care throughout each patient’s journey—admission, midpoint evaluations, and discharge.

• Facilitate patient transfers to higher levels of care when required, coordinating logistics and providing emotional support to both patients and families during transitions.

• Provide crisis intervention services as needed, demonstrating effective management strategies for both the therapeutic milieu and individual sessions.

• Participate in clinical outreach efforts, sharing information about our services and guiding potential clients and families through our programs.

Patient Population / Client Focus\n

You will primarily work with terminally ill patients in a hospice setting, a population that requires a nuanced understanding of grief, loss, and emotional support. The focus of your work will include elderly patients, as well as individuals with chronic and debilitating conditions. You will facilitate coping mechanisms for both patients and their families, helping them navigate emotional distress during a pivotal life stage.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current and active licensure as a Clinical Social Worker (LCSW) in California.

Compensation and Benefits\n

The competitive salary for this position ranges from USD 120,000 to 130,000 per year , commensurate with your experience and case load.
