Key Responsibilities\n

• Conduct thorough virtual evaluations for clients to assess their emotional needs and develop customized treatment plans.

• Engage in therapeutic support through video conferencing, providing clients with a safe space to express their feelings and explore coping strategies.

• Design, implement, and modify individualized treatment plans based on client progress and feedback to enhance therapeutic effectiveness.

• Serve as a guide and advocate for clients, helping them navigate the complexities of hospice care and emotional well-being.

• Collaborate with other mental health professionals and healthcare providers to ensure comprehensive care for clients and their families.

• Document clinical notes in electronic health record (EHR) systems to track client progress and communication effectively.

• Participate in regular team meetings and training sessions to share insights, best practices, and provide peer support.

Patient Population / Client Focus\n

Your primary focus will be on individuals receiving hospice care, along with their families. Many clients face emotional distress, grief, and anxiety concerning terminal illness and end-of-life issues. Your role is to provide them with compassionate support, resources, and coping mechanisms during this significant period of their lives.

Qualifications\n

• A Master's in Social Work (MSW) and an active California Licensed Clinical Social Worker (LCSW) credential.

• Minimum of two years of direct client-facing experience in social work, preferably in healthcare or hospice settings.

• Strong proficiency in using telehealth technology and managing electronic health records with attention to detail.
\n
• Exceptional communication skills, both verbal and written, with the ability to build rapport quickly with clients and their families.

Preferred Experience\n

• Experience working with palliative care or hospice populations, along with a deep understanding of the emotional challenges faced by clients in these settings.

• Familiarity with therapeutic modalities, such as cognitive behavioral therapy (CBT) or mindfulness-based approaches, is advantageous.

Compensation and Benefits\n

This position offers a competitive salary in the range of USD 115,000 – 120,000 per year, commensurate with experience.
