The Bayfront Healthcare Center is actively seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our dynamic team in a full-time remote capacity. This role primarily focuses on providing comprehensive support to clients facing significant mental health challenges, including major mental illnesses and emotional disorders, all while ensuring a patient-centered approach in a hospice care setting.

• Serve as a primary clinician, interacting directly with clients in both office and community settings.

• Develop effective therapeutic relationships to build trust and rapport with clients, essential for successful interventions.

• Conduct thorough assessments to identify symptoms of mental illness and emotional challenges, utilizing evidence-based screening methods.

• Formulate and implement integrated care plans, ensuring continuous access to necessary services and facilitating communication between clients, their families, and healthcare providers.

• Provide a wide range of treatment services, including individual, group, and family therapy, tailored to each client’s unique needs.

• Engage in crisis intervention as required, applying appropriate techniques to support clients during critical moments.

• Maintain accurate and timely documentation of client interactions, treatment progress, and outcomes, along with ensuring completion of required reports and data submissions.

• Participate in team meetings, sharing insights and collaborating on strategies to meet common goals while contributing to agency-wide quality improvement initiatives.

• Provide after-hours on-call coverage as assigned, demonstrating flexibility and commitment to client care.

This role focuses on individuals facing severe emotional and psychological challenges in a hospice setting, often dealing with complex mental health issues. You will work with diverse populations, respecting their unique backgrounds, cultural beliefs, and personal experiences. • A Master’s degree in Social Work (MSW) is required for this position, along with a valid LCSW license.

• Expert knowledge in mental health assessment, diagnosis, and treatment methodologies appropriate for adults and end-of-life care.

• Strong communication skills, adaptable to both verbal and written formats, ensuring clarity and empathy in client interactions.

• Exceptional organizational and planning skills for effective case management and service coordination.

• A minimum of 2 years of relevant clinical experience is preferred, especially in settings that support mental health in hospice or palliative care contexts.

This full-time position offers a competitive salary ranging from USD 70,000 to 90,000 annually, commensurate with experience.
