Work Location: In person Abrazar (Spanish for "to embrace") is a nonprofit direct service organization that has addressed inequities in the social determinants of health since 1975. We embrace Orange County's diverse communities, educate families and individuals about available resources, and empower them to improve their health, stability, and quality of life through culturally responsive, collaborative services. Position Summary Abrazar is seeking an experienced, mission-driven CalAIM Housing Director to lead housing-focused Community Supports for Medi‑Cal members experiencing homelessness, housing instability, and complex health challenges. The Director oversees Housing Transition Navigation Services, Housing Deposits, Housing Tenancy and Sustaining Services, and Transitional Rent referrals. This is a non-clinical leadership position for a housing or social-services professional who is ready to advance beyond a manager-level role. The Director supervises Housing Care Navigators and related staff; translates CalAIM requirements into effective workflows; monitors documentation, billing, compliance, and outcomes; and provides sound leadership in complex member situations. The Director reports to the CalAIM Director and serves as the principal operational leader for CalAIM Housing. Regular fieldwork throughout Orange County includes partner meetings, member-support activities, housing coordination, and landlord and community engagement. Essential Responsibilities Program Leadership and Staff Supervision Direct, supervise, coach, and hold accountable Housing Care Navigators and related housing-program staff. Lead team meetings, case consultations, workflow reviews, onboarding, training, and professional-development activities. Set clear expectations and monitor productivity, service quality, timeliness, documentation, billing readiness, and member outcomes. Manage performance through regular feedback, evaluations, improvement planning, and corrective-action processes in partnership with the CalAIM Director and Human Resources. Cultivate a collaborative, trauma-informed, culturally responsive, and member-centered team environment. Provide operational leadership and continuity in the absence of the CalAIM Director. Program Operations, Compliance, and Improvement Lead day-to-day implementation of CalAIM Housing Community Supports, Transitional Rent referrals, and related housing workflows. Translate DHCS, managed-care-plan, and agency requirements into practical policies, procedures, tools, staff training, and service standards. Oversee CES coordination, referral pathways, placement meetings, housing-match processes, and alignment with Orange County Coordinated Entry requirements. Serve as the program-level HMIS lead or administrator, including user access, data quality, troubleshooting, reporting, documentation standards, and training. Monitor documentation, service authorization, billing support, data integrity, confidentiality, fair-housing compliance, and audit readiness. Review program data, service metrics, housing outcomes, grants, and deliverables; identify performance gaps and implement corrective solutions. Maintain current knowledge of CalAIM Housing Community Supports, Medi‑Cal requirements, CES/HMIS standards, housing resources, and relevant policy changes. Member Support and Service Oversight Guide staff supporting individuals, families, and youth experiencing homelessness or housing instability, including members with complex medical, behavioral-health, or substance-use needs. Review assessments, eligibility determinations, housing plans, service documentation, and Transitional Rent referrals for accuracy, appropriateness, and compliance. Resolve complex, high-acuity, escalated, or time-sensitive situations involving housing access, placement, tenancy, stabilization, and retention. Support housing searches, landlord engagement, move-in coordination, tenancy concerns, benefits access, and stabilization strategies. Maintain a limited caseload or provide temporary member coverage when needed for continuity of operations, while keeping leadership and oversight as the position's primary focus. Care Coordination and Community Partnerships Build productive relationships with landlords, property managers, housing authorities, CES partners, healthcare providers, managed care plans, government agencies, and community-based organizations. Coordinate closely with internal and external ECM teams so housing services align with member care plans, health needs, and broader care-coordination goals. Promote person-centered housing plans that address tenancy barriers, income, benefits, medical and behavioral-health needs, and supportive services. Advocate for members' access to safe, affordable housing and the healthcare, benefits, and community resources necessary for long-term housing retention. Minimum Qualifications Bilingual in English and Spanish or English and Vietnamese. Bachelor's degree in Social Work, Human Services, Public Health, Urban Planning, Public Administration, or a related field. A directly relevant combination of education and experience may be considered. At least three years of progressively responsible experience in housing navigation, homelessness services, case management, care coordination, or a closely related social-services field. At least two years of direct staff supervision or people-management experience in a nonprofit, healthcare, government, housing, or social-services environment. Demonstrated experience setting expectations, coaching employees, monitoring performance, and addressing accountability concerns. Experience serving people experiencing homelessness and/or people with complex medical, behavioral-health, or substance-use needs. Working knowledge of Orange County CES, HMIS, housing resources, and landlord-engagement practices. Strong documentation, organization, case-review, service-coordination, and progress-tracking skills. Ability to protect confidential information and comply with HIPAA and agency requirements. Proficiency with Microsoft Office, electronic records, data-entry platforms, and performance-tracking tools. Ability to work occasional evenings and Saturdays as program needs require. Preferred Qualifications Experience supervising multidisciplinary housing, healthcare, or social-services teams and implementing new programs or workflows. Knowledge of CalAIM Community Supports, the Housing Trio, Transitional Rent, Medi‑Cal systems, and managed-care-plan requirements. Advanced experience with Orange County housing navigation, CES/HMIS, landlord engagement, tenancy support, and housing stabilization. Experience supporting public-benefit applications, including Medi‑Cal, CalFresh, CalWORKs, SSI/SSDI, or related programs. Experience with quality assurance, billing documentation, audits, reporting, grants, and performance improvement. Training in Motivational Interviewing, Trauma-Informed Care, Harm Reduction, Crisis Intervention, De-escalation, Domestic Violence, Suicide Prevention, or related practices. Intermediate or advanced Excel skills and experience with ETO or a comparable case-management platform. Knowledge, Skills, and Abilities Confident leadership and sound judgment, with the ability to mentor staff and make timely decisions in complex situations. Excellent communication, conflict-resolution, negotiation, facilitation, and relationship-building skills. Ability to manage competing priorities, meet deadlines, use data, and adapt to changing program requirements. Strong attention to detail and commitment to accurate, compliant, high-quality documentation. Ability to work independently and collaboratively across housing, healthcare, behavioral-health, and community-service systems. Commitment to trauma-informed, culturally responsive, equitable, and member-centered services and to Abrazar's mission and values. Mandatory Employment Requirements Verification of authorization to work in the United States. Current CPR/First Aid certification or willingness and ability to obtain it. Successful completion of tuberculosis screening, background screening, and drug screening as conditions of hire. Driving Requirements Driving throughout Orange County is an essential function of this position. The employee must have reliable transportation, a valid driver's license, appropriate automobile insurance for any personal vehicle used for work, and the ability to qualify for Abrazar's insurance coverage.
