Responsibilities

• Provide professional nursing care to residents in the long-term care unit, including medication administration and ongoing assessments
• Document care plans, progress notes, and responses to treatment in the electronic medical record
• Conduct routine and focused assessments, monitor vital signs, and recognize changes in patient condition
• Collaborate with the interdisciplinary care team to develop, implement, and update care plans
• Support admissions, transfers, and discharges as needed; communicate with physicians and nursing leadership
• Ensure compliance with safety, infection control, and resident advocacy standards

Requirements

• Active Hawaii RN license
• Current BLS certification
• Minimum of one year of recent Long-Term Care or skilled nursing experience
• Experience with electronic medical records
• Ability to work the assigned schedule, including weekends and holidays as needed
