Work Environment\n

This position is remote and primarily operates within a school facility in Burlington, Massachusetts. While mainly working from home, you will have scheduled visits to clients’ homes and community settings, enhancing your ability to provide personalized care. The positions require adherence to a Monday to Friday work schedule, from 8:00 AM to 4:30 PM, with flexibility for occasional evening or weekend hours as client needs arise. Key Responsibilities\n

As a LICSW at Gotham Enterprises Ltd, you will:

• Conduct comprehensive psychosocial assessments for senior clients, considering emotional, mental, and social dimensions to understand their individual needs.

• Collaborate with clients and family members to develop and implement tailored care plans that reflect specific preferences and objectives.

• Provide counseling, emotional support, and stability to seniors and their families, assisting them with life transitions such as relocation to assisted living or coping with loss.

• Advocate for clients, ensuring they have access to necessary community resources and support services that foster independence and enhance quality of life.

• Work closely with an interdisciplinary team, including nurses, healthcare professionals, and caregivers, to provide holistic care and improve client outcomes.

• Engage in regular home visits to monitor clients' situations, safety, and well-being, documenting insights in electronic health records (EHR) appropriately.

• Resolve conflicts that may arise among family members regarding care decisions, ensuring all voices are heard and respected.

• Educate clients and their families about community resources, support groups, and programs available to promote social engagement.

Qualifications\n

• A master's degree in Social Work from an accredited institution.

• Current and valid LICSW licensure in Massachusetts.

Compensation and Benefits\n
