Key Responsibilities\n

• Serve as a vital liaison between families and outpatient providers, ensuring continuity of care and effective communication.

• Engage with insurance companies for pre-certification processes and participate in utilization management to facilitate client access to necessary services.

• Participate actively in discharge and aftercare planning, working to create comprehensive plans that support ongoing recovery.
\n
• Provide therapeutic meal support, assisting clients in developing a healthy relationship with food and providing guidance on nutrition as part of a holistic care approach.

• Deliver empathetic, affirming care that respects and acknowledges each client’s unique circumstances and intersecting identities.

Patient Population / Client Focus\n

In this role, you will primarily serve older adults facing a range of mental health challenges, including anxiety, depression, and life transitions. Your work will focus on promoting emotional well-being and resilience among clients, guiding them through the complexities of their personal narratives and fostering a supportive environment for healing.

Qualifications\n

• A Master's degree in Social Work from an accredited institution is required.

• A valid LICSW license in the state of Massachusetts must be held.

• Experience in working with older adults is essential for this position.

• Strong clinical and diagnostic skills to assess client needs effectively.

• Excellent communication and interpersonal skills to build rapport with clients, families, and other providers.

Compensation and Benefits\n

The salary for this position ranges from $90,000 to $105,000 per year, commensurate with experience and qualifications.
