Key Responsibilities\n

• Conduct comprehensive assessments of clients’ needs and mental health conditions, implementing appropriate diagnostic tools such as the DSM-5 to inform practice.

• Collaboratively develop and monitor individualized treatment plans that are culturally sensitive and adaptable to address the unique needs of our cancer-impacted clients.

• Maintain accurate and timely documentation of treatment records and clinical information using advanced healthcare technology, complying with regulatory standards and organizational protocols.

• Utilize therapeutic interventions and strategies to assess, evaluate, and support clients throughout their treatment journey, ensuring alignment with patient-specific goals.

• Provide education and mentorship to interns and volunteers, fostering a collaborative learning environment.

• Engage in interdisciplinary team meetings to coordinate care, ensuring that all aspects of patient treatment are addressed comprehensively.

• Manage referrals effectively, establishing strong connections between patients and necessary treatment providers.

Qualifications\n

Compensation and Benefits\n

Community Uplifters LLC offers a competitive salary range between USD 90,000 and 105,000 annually, commensurate with experience.
