Bilingual Case Manager: Housing & Health Advocate
CAMBA
CAMBA, a NYC-based nonprofit, seeks a Case Manager (Bilingual) to support the Scattered Site Housing Program. You will conduct Intakes, Assessments, Service Plans, and referrals, ensuring clients connect to housing, health, legal, and support services.
Responsibilities include documenting interactions, coordinating with partners, visiting clients monthly, and maintaining audit-ready charts while meeting monthly projections and program reporting requirements.
