**Join Our Team as a Rehabilitation Professional in Hawaii!** **Job Description:** Are you ready to embark on an exciting journey in the beautiful state of Hawaii? We are seeking passionate rehabilitation professionals to deliver exceptional care in a Skilled Nursing Facility (SNF) or Intermediate Care Facility (ICF). Your expertise will directly enhance the quality of life for our residents. **Key Responsibilities:** - Provide rehabilitation services in a dynamic SNF/ICF environment. - Conduct evaluations, reassessments, treatments, and other clinical services in line with your licensure and scope of practice. - Collaborate with supervising therapists, physicians, and interdisciplinary teams to develop and implement individualized treatment plans. - Utilize evidence-based therapeutic interventions to maximize function, communication, mobility, swallowing, and overall quality of life. - Educate residents, families, and caregivers on treatment plans and patient progress. - Ensure accurate, timely, and compliant documentation of patient care, progress, and discharges per facility, state, federal, and payer standards. - Supervise therapy assistants, as applicable to your role. - Flexibility to work on Hawaii State and Federal Holidays as required by facility operations. **Required Skills and Experience:** - Minimum of two (2) years in a SNF/ICF setting preferred. **Certifications and Licenses Needed:** - Current Basic Life Support (BLS) certification. - Current State of Hawaii license in good standing appropriate to your discipline. **Shift Information:** - Generally scheduled for five (5) days a week, including one weekend day. - Likely to float to other units or facilities based on census and staffing needs. **Training:** - Comprehensive orientation to rehabilitation department processes, EMR documentation (Net Health Rehab Optima & Epic), and facility workflows provided. **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
